As a Transitional Care Coordinator at Ohman Family Living at Briar, you will play a vital role in the admission process all the way through discharge for our skilled nursing patients.
You will collaborate with healthcare professionals, residents, and their families to develop comprehensive care plans that address their unique needs during the critical transitional period.
Your expertise and coordination skills will be crucial in facilitating effective communication between medical professionals and families to provide continuity of care.
Transitional Care Coordinator
Responsibilities:
Collaborate with hospital discharge planners, social workers, and medical staff to review/approve referrals by assessing their medical history and obtaining insurance authorizations.
Work with IDT to create individualized care plans encompassing medical needs, rehabilitation goals, dietary preferences, and emotional support.
Conduct thorough assessments of residents upon admission, identifying any changes in condition or immediate care needs.
Monitor residents' progress during their stay and adjust care plans as necessary, involving interdisciplinary teams to optimize outcomes.
Provide emotional support and guidance to residents and families during the challenging transition phase.
Maintain accurate and organized documentation of all interactions, care plans, and updates in resident records.
Consult with current and potential families about their financial eligibility for options across the care continuum.
Transitional Care Coordinator
Qualifications:
Bachelor's degree in nursing, social work, or related field of study Active OH licensure in Nursing or Social Work 2 years of experience in healthcare coordination, case management, or discharge planning Alternatively, 3-5 years of experience in long term care admissions, business office, or social services Strong understanding of medical terminology, patient care, and rehabilitation processes.
Excellent communication and interpersonal skills Detail-oriented mindset with the ability to manage multiple tasks and deadline Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite Compassionate and empathetic approach to address the emotional needs of residents and families during transitions Job Type:
